Familial intrauterine convulsions in pyridoxine dependency.

چکیده
Pyridoxine dependency is a relatively rare congenital metabolic disturbance. To date 16 patients have been reported who survived (Schmidt, 1964), and probably 7 of their sibs died because the disease was not recognized in time. The present report concerns the first cases observed in Czechoslovakia. Intrauterine convulsions are even rarer. Only two cases have been described (Badr-El-Din, 1960; Isler, 1964) and one mentioned in passing (Ford, 1960). Only one author (Badr-El-Din, 1960) mentions familial intrauterine convulsions, but there was no suggestion of a connexion between familial intrauterine convulsions and either congenital metabolic disturbance or pyridoxine dependency. For this reason we think our observations will be of interest. The present report concerns three sibs, in all of whom the mother noted intrauterine convulsions: the first two died in status epilepticus. The third survived thanks to pyridoxine treatment, and biochemical studies were undertaken after birth.

Pyridoxine-dependent convulsions in an infant.
The association of dietary pyridoxine deficiency and convulsions was first noted in animal experiments by Chick, El Sadr and Worden (1940). An analogous situation in humans was recognized when Snyderman, Carretero and Holt (1950) described the onset of seizures in a mentally retarded child whose diet lacked this vitamin. Shortly afterwards, an outbreak of convulsions in young infants in the Uni...
متن کاملBranched-chain Amino Acids in Two Mentally Retarded Sibs
There is increasing interest in the importance of pyridoxine in brain disorders of childhood. Convulsions have occurred in infants receiving a pyridoxine (vitamin B6) deficient diet (Coursin, 1955), and Bessey, Adam, and Hansen (1957) have shown that some infants have a greater requirement than others for this vitamin. In the deficiency state, small amounts of vitamin B6 stop the convulsions, b...

متن کاملBenign familial neonatal convulsions: abnormal intrauterine movements, provocation by feeding and ICTAL EEG
An infant with benign familial neonatal convulsions had abnormal movements during the last 2 months of pregnancy suggestive of intrauterine seizures. His postnatal seizures, one of which was captured by electroencephalography, had both partial and generalized features. Most seizures appeared to be provoked by feeding.

A relation between pyridoxine and convulsive phenomena has been established over several years (Snyderman, Holt, Carretero, and Jacobs, 1953; Molony and Parmelee, 1954; Adams, Bessey, Bussey, and Hansen, 1954; May, 1954; Coursin, 1954, 1955). This led to a search for evidence of pyridoxine deficiency among children with cryptogenic epilepsy. Soon patients were discovered who, though receiving a...
